Yep, there re basically 2 questions to answer. Maybe one or both have been answered.
Who added the reporter's name to the group?
Was it deliberate or accidental?
Those answers would determine of someone should be fired or worse.
My understanding is the platform itself was approved unclassified communications under the prior administration. I'd be skeptical of any non-government controlled platform for cabinet head-level discussions, but I can't make a case the platform is insecure.
I have to admit that the odds against an accidental addition to a such a group being a journalist/editor who's openly hostile to the administration are extremely high, but someone does win the lottery periodically, and stranger things have happened.

I've heard there will be Congressional testimony. Obviously, tighter InfoSec procedures are needed and personnel need to be trained. Very reckless behavior.
We should probably also ask if the reporter should have went public, or just notified the government of its mistake. Ethical questions regarding the journalism code of conduct. Did he do unnecessary damage to the United States, or does he have a larger responsibility to hold public figures accountable? Its a debate people should be having, but aren't. The discussion has been 100% political.
Personally, I would not have went public with the breach.
